The following is a booking form to ensure we make a good fit, in terms of an Artist/Client relationship; That we can share a vision on design and application, and that I'm the artist for you.

Please fill out the necessary information below and following questions, upload your reference photo(s), placement photo, and you will be contacted to follow up with booking.
A deposit of $100.00 is required to book, this will cover the cost of setup, medical supplies, tattoo design and is 100% NON-REFUNDABLE.

If you cancel, you will be forfeiting your deposit.
If you need to reschedule, as long as it's within a week or more from the date of your appointment, your deposit is transferable to the rescheduled date.

*** If you're looking to get in for a smaller tattoo (name, quotes or anything of that nature. Small and quick!) contact me direct and I'll try to fit you in sooner than later.
 
 
Please read carefully and provide all the necessary information before submitting an appointment request, and I’ll hope to get you on the books ASAP. Thank You!
 
Things to keep in mind:
  • I prefer black and grey, but will also take on color projects.
  • Cash is preferred; There is a $5 card fee if using a debit/credit card.
  • Appointments always start as early as 12pm by default, my available days/time are Wednesday thru Saturday, 12pm to 7pm.
  • Your tattoo concept/design will be completed the night before or the day of your appointment.
  • The shop minimum is $100 and $100/hr thereafter. Bring potentially $600 to your appointment to safely cover the cost up to a 6 hour session (or however much you're comfortable spending or sitting for). The final cost will be prorated based on the length of our session.
  • Please take note: I'm a very detail oriented artist, so I can't safely say how long each piece will take me to complete. I move at a different pace with different subject matter and/or styles. Some pieces may take several sessions. Your pain threshold also plays a factor.
  • By law, you must be at least 18 years of age to receive a tattoo in the state of Oklahoma. 16 years old with parental consent is not an option, unfortunately.
